ASSIGNMENT:

In line with the above mentioned essential guidelines, complete the following four tasks by way of portfolio evidencing:

Portfolio assessment: Task 1

Write an Essay to demonstrate how personal values and principles influence your contributions to work in a Health and social care setting. In this essay you must make sure that you meet the additional criteria below;-

• A Comparison of your personal values and principles with the principles of support for working in your workplace (1.1 PPD).
• Assess how personal culture and experience influence your role in supporting clients and colleagues in your workplace. (1.2 PPD)
• Discuss how new developments and changes to your personal values can impact on your work (1.3 PPD).(OPPORTUNITIES TO MEET M1, D1, D3)
• Reflect on own personal contributions to the support of individuals experiencing significant life events (3.2. SSLE).
• Make recommendations for improving the support available in a health and social care organization for individuals and their social networks when affected by significant life events (3.3. SSLE). (OPPORTUNITIES TO MEET M3, D1, D3) (Suggested Word count: 1500)

Portfolio assessment: Task 2

You are required to complete a PROJECT to enable you to Monitor, Revise and Evaluate plans for your personal progress in developing the skills and abilities required of a Health and social care practitioner. In this project you should provide – Personal statement and Evidence of work products of the assessment criteria below:-

• Explain own role, responsibilities, accountabilities and duties in the context of working with those within and outside the health and social care workplace (4.1. PP).
• Use of communication skills in a health and social care context (1.2 COM)
• Access and use standard ICT software packages to support work in health and social care (3.1 COM) (OPPORTUNITIES TO MEET M1)
• Evaluate own contributions to place the health and safety needs of individuals at the center of practice (3. HS).
• Evaluate own contribution to the development and implementation of health and social care organizational policy (2. PP) (OPPORTUNITIES TO MEET M1, D3)
• Assess your current skills ability and learning style by using the SWOT Analysis, Honey and Mumford learning style or any other assessment tool (2.1 PPD).
• Produce a holistic development plan for yourself with short, medium and long-term goals (2.2 PPD).
• Monitor your progress against the plan according to the requirements of a care practitioner, revising the plan as required (2.3 PPD).
• Evaluate the effectiveness of the development plan for your development as a Health and social care practitioner (2.4 PPD). (OPPORTUNITIES TO MEET M2, D2, D3) (Suggested Word count: 1500)

Portfolio assessment: Task 3

Write a Personal Statement to demonstrate your understanding of the application of principles of professional engagement with users of Health and social care services. This statement should cover the following assessment criteria.

• Explain the nature of different professional relationships in the context of your workplace (3.1 PPD).
• Evaluate your personal effectiveness in promoting and supporting the rights of the individual in your workplace (3.2 PPD). (OPPORTUNITIES TO MEET M1, D1)
• Discuss ways to resolve issues encountered in professional relationships in your workplace. (3 PPD)(OPPORTUNITIES TO MEET M3, D1, D3) (suggested Word count: 1000)

Portfolio assessment: Task 4

Write a Personal Statement to demonstrate the development of your skills and understanding in relation to working with others in health and social care practice.

• Evaluate the effectiveness of your personal contributions when working with others in your workplace (4.1 PPD).
• Explain how the limits of your work role impacts on work with others (2 PPD).
• Analyze your role in minimising barriers to effective teamwork in your workplace (3 PPD).(OPPORTUNITIES TO MEET M1, D1, D3)
• Discuss how to improve your personal contributions to the collective effectiveness of your team (4 PPD).
• Make recommendations to develop own contributions to meeting good practise requirements (3. PP).
