The present work has been carried out with the sole purpose of intercepting the overall perception of Gold Coast Psychiatric Hospital (GCPH). The respective perception has been entirely based on the method of finding answers to issues that have been raised within this particular given case study. Considering this particular aspect, the context of integrating milieu therapy has also been highlighted in this present piece of work. On the other hand, the present work has been recognized to be entirely based on an analytical approach to the external environments of the respective organization. Finally, the present work has mentioned several important elements that have been described as influential to the external environment of GCPH.
Thesis Statement
The incorporation has enhanced the strategic advancements of GCPH in culture, structure, process, and others.
Discussion and Analysis
External strategic positioning of GCPH in the early days
The hospital “Gold Coast Psychiatric Hospital (GCPH)” did its operation in the same way as it previously did until 1995. It had been catered to the patients who are mentally ill in a process that can be centered on the relationship between the physician and the patients. The treatments are prescribed by psychiatrists mainly in the form of therapy of shock, drugs, and more (Carr and Spandler, 2019). The nurse and other staff of the hospital did their duties and did the maintenance of a variety of necessary functions to keep the hospital in running manner. The treatment also was going well in this hospital for the mental illness. Mainly the difference is being there which is it makes specialization in the treatment of illness mentally as it can oppose the disorder of physiological (Taj-Eldin et al. 2018). In the early days, from 1979 to 1990 year, this hospital was founded as a private hospital or institution. This hospital has a reputation as it gives treatments to mentally ill patients.
It has also the inception and it enjoyed a record of its success. The hospital enjoyed its reputation for using the therapy forms of medical or non-milieu, which was more acceptable or favorable compared to the other hospitals or institutions. The director of this hospital GCPH, Director Richards, had chaired the committee of the hospital for many years. This hospital was respected for its treatment and innovative publications on mental health in the medical area of the field. The hospital is as reputed as other institutions. It helps people with mental treatment to overcome mental illness (Racine et al. 2020). This hospital takes many innovative ideas of treatment and publications in the area of the medical field to treat people with mental illness and mental health. The director of the hospital GCPH was an individual in the community of the hospital whose innovative ideas and medical theories were put into the field of medicine successfully. His ideas and strategy or theory of treatment help and treat people to overcome their mental illness disease.
There is a lot of staff in the hospital and the hospital is full of psychiatrists who are more skilled and experienced. The institution or the hospital GCPH is known for the number of patients who are unusual and received from the hospital staff (Firth et al. 2019). There were no public wards in the hospital means there were no general wards where many patients were kept together. All of the patients are privately treated and privately kept in their ward. The patients were treated by the best physiatrist who is experienced and the patients were also benefited by them from their treatment. There was a dynamic environment in the hospital area. After the early days, society was becoming mature about the problem of mental health during the ’70s and ’80s. The strategy which was used by the hospital GCPH is to give the treatment to the patients in an innovative manner.
They make many innovative ideas as mentioned above and the director of the hospital uses many innovative ideas and medical theories to treat the patients to overcome their mental illnesses situation. The hospital recruited experienced staff and psychiatrists as they can handle easily these situations of mental health (Gopalakrishnan, 2018). The hospital GCPH handles the private patients thus the hospital is under pressure to open the door of the hospital to the public patients. Later the hospital makes it accessible to the public and opens the ward for the public patients. The treatment was used in the hospital exclusively and the experienced professional psychiatrist’s staff and the other staff became overburdened.
A meeting was held in the hospital to make new strategies to handle the problem and everyone agreed that the nurses and the staff, as well as the technicians, play an important role in the field of treatment. The strategies that are taken by the hospital are extremely fit in the external environment as its strategies are acceptable in the treatment of mental health patients. The staff of the hospital had taken a good deal of the work in the hospital only by the psychiatrists (Nochaiwong et al. 2021). However, in the hospital area, there were insufficient resources to train the personnel such as nurses, technicians, and more in their responsibilities. Thus many of the parties agreed that the treatment quality declined during that period. As a result, the hospital was taking many of the strategies as they used innovative ideas more and more and medical theory in the field of mental health.
Impact of milieu therapy to cope with the external environment
Before knowing the impact of milieu therapy, one needs to know what milieu therapy is. The “milieu therapy” is a type of process or method by which the conditions of mental conditions are treated using the surroundings of a person to motivate that person to healthier thinking ways and healthier behavior. In “milieu therapy”, a huge time is spent in an environment such as home, communicating with the people as an individual does his or her world in a day. This therapy can be done in a group or individual (Rageh and Zaher, 2020). The hospital later used this therapy to treat the patients to overcome their mental health conditions. There are many benefits of this therapy and this therapy also benefits the hospital.
This therapy impacted the patient’s mind for treating the mental health conditions. The benefits of this therapy were not specified in the medical field. The treatment of doctor-patient requires a huge number of staff of psychiatrists and thus it is very costly. On the other hand “milieu therapy” also known as “interactive therapy” has been found as more economical for everyone since few doctors could do their deal with many patients (Lakkireddy et al. 2021). Even when there are no psychiatrists or doctors, the patients also communicate or interact with the nurses and other staff technical as well as their equivalents. This therapy is very helpful and economical and also cost-effective for the patients as they have to do only interaction with people about their good and bad feelings to overcome their mental health situation.
The doctors of the hospital GCPH also allow this therapy mostly for treatment purposes. This therapy also impacted the people in the external environment of the hospital. As in this treatment, the patients only have to interact with people, so the external environment is friendlier for the patients. Because in the external environment of the hospital, the patients can be more relaxed and also can interact with other external people except for the nurses and the other hospital staff (Stefanovic et al. 2020). This treatment was also applicable to people who are not admitted to the hospital or who are living in the external environment of the hospital. Most people are suffering from this problem of mental health, but with this treatment, this condition is easily resolved. Thus this treatment impacted also the external environment of the hospital. At the hospital GCPH, there have been developed two groups of thought psychiatrists.
One group of psychiatrists was allowing the traditional approach of therapy for the treating purpose. On the other hand, the other group of psychiatrists maintained the approach of “milieu therapy” as they believed that the entire group of patients, as well as the external environment, benefited from the therapy of “milieu therapy” (Zhang et al. 2018). Drs. Theoret and Gosselin, who are the recruits in the hospital, believed that the hospital GCPH could use its resources to utilize the therapy of “milieu therapy”. Theoret and Gosselin thought that “vested psychological interests on the part of the conservative staff to treatments which they helped to develop” and claimed that the committee of the hospital took an approach for the treatment. The doctors of the hospital worked on the “milieu therapy” and applied this in England and saw it successful in the patients. Later the hospital hired new three staff and each of them was familiar with the therapy.
Later, the staff of the hospital refused to implement the “milieu therapy” and Dr. Stephenson, Gosselin, and Theoret, who are the recruits in the hospital, and also the three new staff the hospital defeated to introduction of the “milieu therapy”. The “milieu therapy” is very important to cure the problems of mental health by communicating with the people. This therapy is very effective in bringing down the conflict behavior of the patients with the illness mentally. The patients who are receiving the treatment are facing challenges to develop their values as well as maintain their values during the “milieu therapy” (Bajgain et al. 2018). This therapy is based on some components which are support, validation, structure, containment, and involvement. There are many advantages of “milieu therapy”. The advantages are it gives a sense of security, it gives them the freedom to people from a system of construct, it gives a supportive environment, and more. The hospital GCPH also takes this therapy as the treatment of the patients private or public and the impact of this treatment is spread over the internal environment as well as the external environment of the hospital.
Importance of internal elements within the external strategic planning
The case study has given evidence about the importance of milieu therapy that has been recognized within the periphery of psychology therapy throughout the world. According to the research by Nåvik et al. 2020, milieu therapy (MT) has been described as one of the most complicated methods having the ability to integrate “strategic utilization”. It has been recognized to be based on the implementation of a “psychological milieu” within various institutions, communications, and relations that have been distributed among therapists, nurses, and other associates. Considering this particular context, it has been pointed out by the given case study about “financial resources” as an important element that plays a pivotal role in the implementation of milieu therapy. This particular context has been mentioned by the given case study as it has been entirely merged with a “stopgap approach” that has been integrated to raise funds. This particular aspect is linked to the structural changes that have been mentioned in the case study.
It has been described within the given case study as “a change in personnel” that has been considered to be associated with the respective therapy. It has been highlighted in the case study that two of the specialists known for working with dynamism at a British psychiatric hospital have taken the initiative to work on milieu therapy. As suggested by Magnowski and Cleveland (2020), this particular aspect can be analyzed because milieu therapy has been recognized for using the fundamentals of ethos. As described by Ratini (2021), the idea seems to be highly effective in providing treatment to shape mainly the “problematic behavior”. Following this particular context, the given case study has highlighted the importance of the supportive structure that has been developed by the associates of GCPH. The doctors that have been appointed in charge of this facility have been recognized to be able in terms of building a “lenient environment” that eventually helps to cope with this particular environment.
Considering the internal element, another fact can be pointed out that, the staff and the doctors have been recognized to impose the practice of an “individualistic approach” to provide comfort to the patients of this special facility or therapy. Contrarily, the case study has mentioned the administration of the respective organization has faced an enormous hurdle in terms of implementing this particular style of therapy within the institution. It is linked to the “unconscious use” of this particular therapy by the staff way before the decision has been taken. The strategic decisions that have been taken to deal with these issues have been recognized to be more focused on conducting “group interviews” to explore the overall experience of the staff members (Kanellopoulos et al. 2021). Table 1 has highlighted the internal elements that play a pivotal role within the context of integrating milieu therapy. These elements have also been considered to be attached to the conceptual framework for the respective organization.

Culture has been mentioned as an important element of GCPH’s internal elements as it is linked to the changes that have been imposed by the respective context. This particular aspect can be described as Milieu therapy is based on several characteristics linked to the cultural aspect. The fundamental idea has emerged from the absolute desire to handle the “regressive effects” that have been imposed by the means of “initialization” (MENTAL HEALTH NURSING, 2020). This particular aspect can be interpreted as the independent implementation of this particular therapy that needs to be carried out by the respective staff. According to the case study, the cultural aspect of this aforementioned organization in its early days changed drastically after the decision was taken for the therapy (Ward-Miller et al. 2021). As the financial aspect is also concerned, it can be derived from the observation that “adaptation of the environment” can play a central role in affecting the structural aspect of GCPH. As the therapy needs to be nourished by a proper distribution of power within a certain organization, it has changed the core of the entire system of GCPH.
Conclusion
The present work has been carried out to intercept the detailed aspects of impacts that have been generated by milieu therapy. The work has introduced several general considerations that have been based on the psychological aspect of milieu therapy. This present work is entirely based on GCPH as the given case study has mentioned. This respective organization has been facing some changes that have been generated as the decisions have been taken. This particular piece of work has highlighted the changes before and after the incorporation of therapy. Additionally, it has extracted several internal elements that have been mentioned by the topmost authority of GCPH being highly influential for the organization. The elements have varied from a range of cultural aspects to the structural changes that have been imposed on the organization. On the other hand, the present work has identified the importance of these factors in terms of motivating the entire workforce for GCPH to cope with the current strategic consideration of GCPH.
